Woman charged with forging an oxycodone prescription

FREEPORT, Ill. (WIFR) A Chicago woman is served with two felony arrest warrants after police say she forged an oxycodone prescription in Freeport.

23-year-old Carin Alvarado was served the warrants while in the Stephenson County Jail. Police say in December of 2017, officers were called to the CVS in the 1500 block of W. Galena Avenue for a fraudulent prescription.

Police discovered Alvarado used another person’s driver’s license and insurance card in an attempt to fill the prescription. Investigators say on that same day, Alvarado showed the counterfeit prescription to another pharmacist pretending to be someone else.

She’s charged with unauthorized possession of prescription form, unauthorized acquisition of a controlled substance, unauthorized use of medical assistance and identity theft.
