The Register Star says a team of case workers were sent into The MILL Friday following an ongoing investigation of the facility.
In March, specialists from the University of Illinois Chicago were sent into the youth home after DCFS heard of an unreported suicide attempt.
The Register Star says other complaints of the facility include "unacceptable levels of violence and failure to report shortcomings to the state."
The MILL is a 26-year-old agency that treats troubled youth for psychological and behavioral disorders. There are 50 teenagers at the facility.
