December 9th, 2009 (Rockford, Illinois)...The Service Collaborative team at OSF Home Care Services has focused on improving service success with patients and families. The collaborative has developed service cards to be given to patients on every visit, modified admission booklets to focus on service mentality and created two Traveling Angel Awards to be recognized each quarter.

One award is given to the department with the most improved Press Ganey score and the other award to the department with the highest score among OSF Home Care Services.

This quarter, the hospice team in Rockford earned both awards – a first for OSF Home Care Services! The hospice team ranked in the 99th percentile among their peers for having the Highest Overall Mean Score (97.8).

The hospice team also received a Traveling Angel Award for the Most Improved Agency for the quarter, going from the 6th to the 99th percentile (improving by 7.6 points).

The Traveling Angel Awards and plaques are on display in the Rockford office. Congratulations!
