(AP) -- Illinois has launched an online survey to gather information about diabetes.

The "Speak Out About Diabetes" survey is available in English and in Spanish on the state Department of Human Services Web site. Results will be used to improve diabetes prevention and control.

The survey includes questions such as:

--"Are there things that prevent you from being physically active each day?"

--"Have you ever seen a dietitian about how to manage your diabetes?"

In Illinois, more than 841,000 adults have been told by their doctor they have diabetes.

The survey is a project of the Department of Human Services and the state Diabetes Commission. The commission was established by law to identify additional diabetes needs across the state.
