investigation, which revealed that the applicant was convicted of a Class 2 felony theft in June, 2003.

Roger Palmer, De Kalb – associate appraiser license reprimanded and fined $2,000 after submitting an appraisal report to a client without having a certified appraiser co-sign it and committed substantial violations of the uniform standards of practice.

indefinitely suspended for failure to file and/or pay Illinois state income taxes.
Stephen A. Freeman, Rockford – cosmetologist license (011app859848)
issued and placed on probation for one year due to a criminal conviction.

Kimberly R. Jenkins, South Beloit – permanent employee registration card (129235398) revoked due to felony conviction for manufacture/delivery of controlled substances and failure to report arrest and conviction; defaulted on complaint.
Timothy H. Christianson, Sterling – physician and surgeon license (036081446) placed in refuse to renew status after being disciplined in the state of Colorado.
Sheilah Dye, Rockford – registered nurse license (041311188)
restored to probation for three years.
Leslie Hecht-Cousin,Loves Park – registered nurse license (041248574)
indefinitely suspended for failing to appear for drug toxicology testing and failure to submit monthly reports.
Tiffany P. Harris, Rockford – licensed practical nurse license (043082764) placed on indefinite probation after defaulting on an Illinois educational loan and has now entered into a repayment agreement.
Harold Jerde, DeKalb – real estate salesperson license cancelled and refused renewal for two years for falsifying his 2005 renewal application by certifying that he had complied with all continuing education requirements.
Joel Patch and William Mears, Janesville, WI – Joel Patch assessed a $10,000 fine for engaging in the unlicensed practice of real estate and William Mears was assessed a $10,000 fine for aiding and abetting the unlicensed practice of Mr. Patch.
