The accident happened on Route 2 and Old River Road around 11 a.m. Tuesday morning.
Winnebago County sheriff's police say a Cadillac and a Mercury Sable collided head on. Extrications were needed for both vehicles.
Police say one of the victims may have suffered a heart attack. At least two people were taken to an area hospital.
Police do not believe their injuries are life-threatening.
